KMGSA: The Kitchener Minor Girls Softball Association is committed to protecting the privacy of your personal information. Parental Information: The Kitchener Minor Girls Softball Association (KMGSA) provides amateur softball programming in the City of Kitchener, in cooperation with the Grand Valley League (GVLSL), the Provincial Women's Softball Association (PWSA), Softball Ontario, and Softball Canada. To maintain current programs, KMGSA collects information for the purposes of selecting and certifying coaching staff, registering players, establishing residency and age requirements, facilitating emergency contact and treatment, providing skill development, practice, game and tournament opportunities for their players. KMGSA will collect only information necessary to conduct softball programming, and is committed to protecting the privacy of this information.

Types of Personal Information		Purpose of Collecting
A participant’s name, gender, place of residence, date of birth, birth registration #, school attended		To establish age, gender and residency compliance for local association, league, tournaments and provincial association
Doctor and phone number, health concerns, emergency contact		To facilitate medical treatment, ensure safety of participants
Parent/guardian information, name, address, phone numbers		To provide emergency contact, consent for participation of minor registrant
Email addresses		To facilitate membership communication, team communication
Historical record of previous teams played for, development level, honours, awards		To determine residency requirements, monitor success of our programs
Coaches certification information, resumes or applications, evaluations, disciplinary records		To aid in selection and management of coaching staff
Sizing information		To facilitate provision of uniforms, jackets, other equipment and accessories
Statistics such as batting average, at bats, positions played, etc		To allow coaches to improve and develop individual and team performance

*IMPORTANT* The act of registering your child with KMGSA constitutes implied consent to the use of personal information for that year. If you wish to withdraw your consent at any time, you may do so by contacting KMGSA and providing reasonable notice.
